The standard for hypertension by WHO: adults with systolic pressure >/= 21.3 kpa (160mmHg) and diastolic pressure >/= 12.7 kpa (95mmHg) is diagnosed with hypertension. Manifested the neural dysfunctional syndrome with the pathogenic changes of heart, brain, and kidney in the late stage. Hypertension is closely related to the morbidity and mortality of the cardiovascular and cerebrovascular diseases. If the disharmony between the left ventricular output and the peripheral vascular resistance exists, the increasing, no matter the output or the resistance or both, will finally result in hypertension.
Aetiology may relate to the following factors:
(1). Higher nervous centre dysfunction
(2). Renal small arteries spasm and ischemia, excess secretion of renin, renin-angiotensin-aldosterone system hyperactivity (RAAS)
(3). Genetic factor
(4). Environmental factor
(5). Endocrine factor, cerebral cortex dysfunction resulting in (a). the dysfunction of hypothalamic neurosecretion and the increasing of pitressin released from lobus posterior of pituitary, (b). the increasing of corticotropin-releasing factor secreted from hypothalamus and adrenocortical hormone (ACH), (c). the small arteries spasm due to the excitement of sympathetic nerve, and the increasing of adrenalin and noradrenalin secreted from adrenal medulla.
(6). Abnormal transportation of Na+, Ca++ ion on cellular membrane.
(7). Induced by some medications such as nonsteriod anti-rheumatoid drugs, oral contraceptives.
Aetiology and pathogenic explanation in TCM:
Emotional injury, improper diet and internal damage, resulting in imbalance of yin and yang, especially, the imbalance of yin and yang in the kidney and liver, insufficiency of kidney yang, up-flare of liver, deficiency of the lower jiao and excessive upper jiao, deficiency of yin and yang or sudden hyperactivity of the liver yang, even stroke and syncope with prolonged course.
