Chronic prostatitis can be divided into two groups: bacterial one and non-bacterial one. The former is infected by bacteria, which invaded to prostates through urethra or blood or lympha circulation. The latter is caused by continuous congestion in prostates, especially the passive congestion. Congestion often happens with abnormal intercourse, such as excessive intercourse, interrupted intercourse, frequent masturbation, or orgasm without ejaculation. Direct pressed perineum such as riding bicycle or horse and drinking may cause chronic prostatitis.
Pathology:
Prostatic capsule, glandular duct and interstitial inflammation, finally the body of gland structure is destroyed, shrunk to fibrosis, prostates shrunk, fibrosis invaded to posterior urethra making the neck of prostates harder. The walls of seminal vesicle and vas deferens become thicken with the fibril tissue proliferation, seminal vesicle and the opening of ejaculatory duct become stenosis with the fibrosis.
Symptoms:
1. Urinary symptoms: Frequent and urgent micturition, pain or burn feeling with urination radiating to the head part of penis, mucous or pus dropping from the opening of urethra in the morning, sometime there is bloody urine at the end, even difficult urination or retention of urine.
2. Local symptoms: Generally pain is not severe and located unclearly. Posterior urethra, perineum and anus feel uncomfortable, pain, heavy, distention and worse when squat down and bowel movement.
3. Radiating pain between diaphragm and knees, more common at the lumbar area, sometimes radiated to the penis, testicle, and groin, above pubic region, inner of tight and buttock.
4. Sexual dysfunction: lower sexual drive, pain in ejaculation, hemospermia, impotence, infertility, white mucus drops from the opening of urethra at the end of urination or bowel movement.
5. Mental symptoms: Anxiety to this disease, fatigue, dizziness, insomnia, and depression.
Pathology explained in TCM:
Deficiency of Yin and fire up, damp heat downwards, deficiency of kidney Qi, and meridians blocked by blood stasis.
